A controlled study of the effects of leucotomy.

چکیده
Previous Studies Although the operation of leucotomy is now over 20 years old only a few attempts at a controlled assessment of its therapeutic effect have been made (Penrose, 1944; Mettler, 1949, 1952; Friedman, Moore, Ranger, and Russman, 1951; Livingston, 1953; Jenkins and Holsopple, 1953; Freeman, Davis, East, Tait, Johnson, and Rogers, 1954; Jenkins, Holsopple, and Lorr, 1954; Medina, Pearson, and Buchstein, 1954; Powell, 1955; Crandell, Zubin, Mettler, and Kugelmass, 1956). Of these, moreover, a number have suffered from either (a) a poor matching of control and treatment groups (e.g., Mettler, 1949; Friedman et al., 1951; Freeman et al., 1954; Powell, 1955) or (b) too short a period of follow-up (e.g., Penrose, 1944; Mettler, 1949; Jenkins and Holsopple, 1953; Livingston, 1953; Freeman et al., 1954; Jenkins et al., 1954) so that their validity is suspect. Finally, the conclusions of these studies are conflicting. While intramural improvement is generally reported (Mettler, 1949; Friedman et al., 1951; Jenkins and Holsopple, 1953; Jenkins et al., 1954; Medina et al., 1954) di,charge rates are reported as improved by some (Penrose, 1944; Freeman et al., 1954; Friedman et al., 1951), as unchanged (Mettler, 1949; Jenkins and Holsopple, 1953; Medina et al., 1954; Crandell et al., 1956), or even as worse by others (Powell, 1955).

Controlled multifocal frontal leucotomy for psychiatric illness.
There have been two important developments in the surgical treatment of mental disorders during the last 12 years, and one of these is in the selection of patients. No longer is the chronic schizophrenic the principal type of patient selected for cerebral surgery. The patients now considered for surgical treatment are usually chronic depressives and those with anxiety states, and obsessional sy...

Background: Traditionally some people employ Ocimum basilicum (Shaspram) to relieve the symptoms of dyspepsia. We therefore studied the effects of oral extract of this medicinal plant on functional dyspepsia. Methods: In a double-blind placebo-controlled clinical trial, the effect of a four-week treatment of Shaspram was evaluated on functional dyspepsia. Two hundred cases from all patients ref...
متن کاملThe metabolic effects of limbic leucotomy in Gilles de la Tourette syndrome.
Regional cerebral oxygen metabolism was measured before and after limbic leucotomy in a patient with Gilles de la Tourette syndrome, obsessive compulsive disorder, and obsessional slowness. The preoperative scan showed hypermetabolism in the caudate nuclei, which normalised after operation. It is proposed that the beneficial effects of this operation on both tics and obsessive compulsive behavi...

Little has been published concerning the occurrence of delayed operative death after bilateral prefrontal leucotomy. The term " delayed operative death" in relation to leucotomy was introduced by Meyer and McLardy (1948; 1949) in analysing undesirable clinical sequele suffered by patients submitted to leucotomy who had survived the danger of fatal operative haemorrhage, and whose brains had in ...

Preliminary observations (Elithom, Piercy, and Crosskey, 1954 a and b) on patients undergoing prefrontal leucotomy revealed changes in autonomic function following the operation, and it was suggested that these were largely due to a reduction in the abnormal autonomic activity which accompanied the pre-operative mental disturbance. Similar views have been expressed by other workers (Carscallen,...
